Dorval Medical Associates Family Health Team (DMAFHT) is a non-profit Family Health Team located in Oakville, Ontario. Founded in 2007, we have been serving our community for 20+ years, providing comprehensive primary care to over 7,000 patients.

Our team of physicians, nurse practitioners, nurses, and allied health professionals work side by side to deliver coordinated, patient-centred care — all under one roof.

Urgent & Emergency Situations

I am experiencing shortness of breath or breathing problems. What should I do? 

Shortness of breath and breathing difficulties can be serious. Please go to your nearest Emergency Department immediately or call 911. Our clinic is not equipped to manage cardiac or respiratory emergencies. 

What should I do in a medical emergency? 

In any medical emergency, call 911 immediately or go to your nearest Emergency Department. Do not wait for a clinic appointment for life-threatening conditions. 

What should I do when the office is closed and I have an urgent, non-emergency concern? 

You can contact Telehealth Ontario at 1-866-797-0000, available 24 hours a day, 7 days a week, to speak with a registered nurse. For emergencies, always call 911 or go to your nearest Emergency Department. 

Mental Health Support

I need mental health support right now. What should I do? 

If your mental health needs are a medical emergency, or there is a concern you might hurt yourself or someone else, contact 9-1-1, or go to your nearest hospital emergency department.

What if I am looking for mental health support?

Please book an appointment with your doctor to discuss your mental health needs. Your physician can support your mental health care. Other options may include speaking with our Registered Social Worker, who specializes in mental health, providing you with community resources or referring you to specialized services.

How to get mental health support in Halton

Crisis Outreach And Support Team (COAST) provides Halton residents 16 years and older, immediate support when experiencing a mental health crisis. Partnered with Halton Regional Police, COAST has a mobile response team who can visit you to help provide short-term support and keep you safe.

About our Practice

What is a Family Health Team (FHT)? 

A Family Health Team (FHT) is a model of primary care in Ontario that brings together family doctors, nurse practitioners, nurses, social workers, pharmacists and other health professionals to provide comprehensive care to patients. Rather than seeing just one provider, patients benefit from a whole team working together to support their physical, mental, and social health needs. Dorval Medical Family Health Team is fortunate to also have an on-site lab available to registered patients and an on-site pharmacy.

FHTs focus on keeping people healthy, managing chronic diseases, and making sure patients have access to the right care at the right time — all in one place.

We are proud members of the Association of Family Health Teams of Ontario (AFHTO) (insert live link to AFHTO website), a province-wide organization that represents and supports FHTs across Ontario. AFHTO advocates for strong, community-based primary care and helps teams like ours share best practices, improve care quality, and better serve our patients.

Are you accepting new patients? 

We are not currently accepting new patients, however many clinics around us are. For assistance in being connected with a Family Doctor, visit www.supporthouse.ca and click on "I need healthcare support or to find a family doctor. You can also visit https://www.ontario.ca/page/find-family-doctor-or-nurse-practitioner and be connected with a family doctor or nurse practitioner.

What are your hours of operation?

Our office is open Monday to Thursday from 9:00am-8:00pm, Friday from 9:00am to 5:00pm and Saturdays from 9:00 a.m. to 12:00 p.m. by appointment only. Phones are answered beginning at 8:30 a.m on Saturdays. Weekend appointments are reserved for urgent concerns only.  

Do you use Artificial Intelligence (AI) in your clinic? 

Yes. Our physicians and some healthcare providers use AI-assisted tools to support clinical documentation. These tools help your provider focus more time on you during your visit. All notes are reviewed for accuracy, and all care decisions are made by your provider(s). 

If you have questions or concerns about how AI is used in your care, please speak with your physician. 

Appointments

Can I book an appointment online? 

Yes. Online booking is available for same-day appointments with our Nurse Practitioner (NP), our Lab Technician, and select nursing appointments. For all other appointments, please call our office directly. 

Do you have same-day appointments? 

Same-day appointments are offered by a Nurse Practitioner. You can check availability and book online as of 8:30am or call the office beginning at 9:00am for an appointment on Mondays. Wednesdays, Thursdays and Fridays. 

How do I cancel an appointment?

If you need to cancel, please leave a message on our cancellation line as soon as possible. We ask that you give as much notice as you can so that your appointment time can be offered to another patient. Please note that these messages are not returned. You may also cancel your appointment by e-mailing [email protected] or by clicking the cancel appointment link on the appointment reminder you may have received by e-mail after booking. 

I am running late for my appointment. What should I do? 

Please call the office immediately to let us know. We will do our best to accommodate you; however, late arrivals may result in your appointment being rescheduled. 

My family doctor isn't available. Is there anyone else I can see? 

Yes. Please call the office or visit our website for information about same-day appointments with our Nurse Practitioner. Our NP can address a wide range of health concerns and will ensure continuity of your care. 

Do you offer virtual or telephone appointments? 

Virtual and telephone appointments may be available for certain types of visits. Please call our office to find out whether your concern can be addressed this way and to schedule accordingly. 

Prescriptions & Referrals 

How do I get my prescription refilled? 

Prescription renewals require an appointment with your family doctor. We recommend booking before your medication runs out to avoid any interruption in your treatment. Same day nurse practitioner appointments are not for prescription refills. 

I will run out of my medication before my next appointment. What should I do? 

Your medication is your responsibility. Refills are generally given for several months, when appropriate, giving you ample time to book a follow up for refills. If you will run out of your medication before you can see your family doctor, please speak with your pharmacist, who may be able to provide a bridge supply. 

How do I request a referral to a specialist? 

Referrals to specialists are arranged by your family doctor. Please book an appointment to discuss your concern, and your doctor will work with you to determine whether a referral is appropriate and submit the necessary paperwork. 

Lab Services & Test Results 

Do you have a lab on-site? 

Yes, we have an on-site lab for registered patients only. Appointments can be booked online at by clicking here or by calling our office. 

I am waiting on test results. What should I do? 

Please book an appointment with your doctor to discuss your results. Your physician will review them with you and advise on any next steps. 

Can you email me my test results? 

Your test results may be shared with you electronically via Medeo after you have spoken with your doctor. Please book an appointment to review your results first, and your doctor will advise on how they will be communicated to you. 

Services & Fees 

What services are not covered by OHIP, and what do they cost? 

Some services are not covered under the Ontario Health Insurance Plan (OHIP). A complete list of uninsured services and their associated fees is available on our website here. If you have questions about a specific service, please contact our office. 

What other services do you offer? 

In addition to primary medical care, our clinic offers a range of services including social work, minor procedures performed by select physicians, nursing services such as vaccinations, stitch removal, TB testing and more. We also have an on-site lab and pharmacy. Please contact the office or see our website here for a complete list. 

Do you offer preventive care and health screenings? 

Yes. Preventive care is an important part of what we do. Our team provides immunizations, health screenings, and routine checkups. Please book an appointment with your physician or nurse to discuss your preventive care needs. 

Forms, Notes & Fees 

I need a form completed or a medical note. What should I do? 

Forms and medical notes require an appointment with your doctor. Please book an appointment to discuss your needs, and your physician will complete the required documentation. 

Is there a fee for medical notes and forms? 

Yes. Fees apply for medical notes and completed forms. The cost depends on the type and complexity of the documentation required. Your doctor will advise you of the applicable fee at your appointment. Fees for uninsured services can be reviewed here.

Clinic Facilities & Parking 

Is there a pharmacy nearby? 

Yes! Total Health Pharmacy is located right within our clinic. Our pharmacy team works closely with our medical staff to support your care and make filling prescriptions as convenient as possible. 

Do I have to pay for parking?

No. Parking is free of charge for all patients and visitors. 

Can I check in online vs visiting reception? 

Yes! We have a convenient self-check-in kiosk in our waiting room. 

Your Health Card & Personal Information

I received a new health card. Do I need to let you know? 

Yes, please inform us as soon as possible. We will need your updated health card number, the expiry date (found in the bottom right-hand corner of the card), and the two-letter version code that follows your 10-digit health card number. 

How do I update my contact information? 

It is important that we have your current address, phone number, and emergency contact on file. Please call the office or speak with our front desk team to update your information. 

Can I request a copy of my medical records? 

Yes. You have the right to access your personal health information. Please contact our office to submit a request. A consent form will need to be completed, and processing fees apply. Records can be provided on an encrypted memory stick or printed, with the memory stick being the most economical route. 

Privacy

How do you protect the privacy of my medical information? 

We take your privacy very seriously. Your personal health information is protected under Ontario's Personal Health Information Protection Act (PHIPA). We will not release any medical information without your explicit written consent, except as required by law. All staff are trained in privacy practices and bound by strict confidentiality obligations.
