About Family Health Teams
The Ontario Ministry of Health and Long Term Care created 185 Family Health Teams to better meet the primary healthcare needs of Ontario’s residents. In Family Health Teams, family doctors work with other health care professionals such as nurse practitioners, nurses and mental health counselors to provide more effective, coordinated and comprehensive care to their patients.
About Dorval Medical Family Health Team
Dorval Medical Family Health Team is a family practice consisting of six family doctors, two Nurse Practitioners, three Registered Nurses, one RPN, a mental health counselor, a pharmacist and a dedicated staff. The practice is focused on fully coordinated, comprehensive and continuity of care through:

- Full integration with the Community Care Access Centre
